Form I is now called “Committee Nomination, Conflict of Interest Disclosure, and Updates.” This creates a graduate student’s Doctoral Committee and is processed online.
 
Form II is now called “Report on Advancement to Candidacy for the Degree of Ph.D. or DMA.” This is completed after a graduate student’s prospectus has been approved and is processed online.
 
Form III is still listed as “Doctoral Degree Form III – Report on Doctoral Degree Final Defense (Public or Waived). File a Form III when a graduate student’s dissertation is completed and approved.
 
Conflict of Interest Form COI:  This is a stand-alone form used to report on possible conflicts of interest in graduate education. It is usually completed with the Committee Nomination.
